Your First Visit

A  bit of paperwork  

We will need to fill out forms that provide general information about you and your condition. Our staff is happy to assist you with the paperwork. In order to save you some time, we have included the Registration documents at the bottom of the page. Please feel free to download and print them so that you may fill them out at your convenience. 

Consultation
During your first consultation with our professionals, you'll discuss any health-related issues and concerns. You will also learn about potential treatment options.

History and examination
In order to pinpoint the problem, we will ask you a number of questions related to your condition. 

Same-day treatment
Patients will generally undergo their first treatment on their first visit after the examination.

Home instructions
Before leaving, you will be given instructions on certain activities or procedures to be conducted at home. This may include avoidance of certain activities or positions, as well as home exercises and dieting.

Scheduling the next appointment

Regular treatment is essential to the process. We work our treatments around your schedule. Treatment options for your condition will also be discussed at this time.

Balanced Qi Acupuncture accepts most insurance providers. We also accept self-pay to help you have a hassle-free financial experience.



Work compensation and Auto accident claims are also accepted.

We want to answer as many of your questions at your visit.
Please prepare for this by bringing:

• Health Insurance Card/Information

• If needed, a friend or family member

• A list of all medications you are taking

• A list of your questions /concerns

• Copies of records from doctors related to your visit with us.
